Special Projects Case Manager Overview
Under the supervision of the Project/Program Manager II, the Special Projects Case Manager will be well versed in HAY Center services and provide case management support to current and former foster youths and young adults between the ages of 16 and 25 as they work towards self-sufficiency. The position will be assigned specific job assignments for designated time periods to best support HAY Center programs.
Duties & Responsibilities- Wraparound case management: provide intensive case management to address current and former foster youth's transitional needs by engaging in program intakes, case planning, self‑sufficiency assessments, wraparound meetings, and face‑to‑face contact with clients on a consistent basis.
- Workforce case management: support HAY Center youths and young adults as they navigate employment training programs.
- Educational case management: assist with educational events (College Tours, College Bound Experience, College Fairs, etc.).
- Housing case management: help youths and young adults secure and maintain stable housing.
- Teach state‑mandated life skills curriculum via classroom setting during peak seasons (typically Spring Break and Summer) in six core curriculum areas.
- Participate in and attend HAY Center events and regional activities as required.
- Bachelor’s degree in Education, Psychology, Sociology, Social Work, Criminal Justice, or another Behavioral or Social Science.
- Minimum of two (2) years full‑time paid case management experience working with youth in a social service setting.
- Experience using a personal computer with Microsoft Word, Microsoft Excel, and Case Management Software.
- Reliable transportation with a valid State of Texas driver’s license and auto liability insurance.
- Ability to become CPR certified.
- Knowledge of adolescent/transition‑age youth developmental stages.
- Problem‑solving and critical thinking skills.
- Ability to maintain confidentiality.
- Professional working relationships with individuals of varying backgrounds.
- Team‑work and goal‑orientation.
- Case management database proficiency.
- Clerical skills.
- Follow written and oral instructions.
- Self‑disciplined, independent, responsible, and self‑motivated.
- Excellent time management.
- Ability to drive a 15‑passenger van.

- Monday – Friday 8:00 a.m. – 5:00 p.m. or Monday – Friday 9:00 a.m. – 6:00 p.m. (Some Saturdays)
- Based on 26 pay periods.
- Plus mileage reimbursement.
Employment is contingent upon passing a background check and drug screen.
