** Job Summary
** The Case Manager participates in outreach activities to identify and enroll SSVF-eligible veterans in case management services, ensuring timely access to safe, permanent housing and essential supportive services. The position utilizes a
** Housing First Model
** to develop and implement
** Housing Stability Plans
** that promote household stability and self-sufficiency.
** Key Responsibilities
*** Conduct
** full assessments
** with applicants to enroll SSVF-eligible veterans in case management for a minimum of 90 days, documenting all activities and events
* Ensure
** rapid entrance into permanent safe housing
** or maintain stable housing to prevent homelessness
* Mediate, advocate, and leverage support for eligible veterans, referring them to healthcare navigation, legal, employment, financial, and other services as needed
* Identify and connect veterans to
** community resources
** for overall well-being, including emergency housing needs
* Maintain
** accurate records
** of all resources provided, including temporary financial assistance, and manage client files safely while preserving confidentiality
* Represent Baker Ripley and the Veteran Services team professionally in internal and external meetings and events
* Collaborate with other agencies to provide a comprehensive case management process
* Perform other duties as assigned## Qualifications
** Education
* * Bachelor’s degree preferred; high school diploma or GED required
** Experience
* * 2+ years of case management experience; 1–3 years minimum; 2+ years social services experience preferred
** Skills
* * Proficiency in Excel, clear and accurate correspondence/reporting, ability to work in a professional, client-focused environment
** Licenses/Certifications
** Valid driver’s license; experience working with veterans or in similar outreach roles is a plus
** KSAs
* ** Ability to develop Excel spreadsheets using formulas
* Ability to write clear and accurate correspondence and reports
* Ability to effectively present information and respond to questions both orally and in writing in a manner that is understandable to others
* Ability to calculate figures and amounts
* Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form
* Excellent interpersonal skills and successful experience working in teams
