The Social Worker/Case Manager supports individuals and families in accessing resources and services to address their social, emotional, and economic challenges. This role involves assessing client needs, creating care plans, and advocating for client well-being.

Key responsibilities include:

• Conducting assessments to identify client needs, challenges, and goals.
• Developing individualized service or care plans.
• Providing counseling, emotional support, and crisis intervention.
• Connecting clients with community resources such as housing, healthcare, and education.
• Advocating for clients in securing necessary services and benefits.
• Monitoring and documenting client progress and updating plans as necessary.
• Maintaining detailed and confidential client records.
• Collaborating with other professionals and agencies to provide comprehensive support.

Requirements:

• Bachelor's degree in social work, psychology, or a related field (Master's preferred).
• Minimum of 2 years of experience in case management or social work.
• Strong understanding of social service systems and community resources.
• Excellent organizational and interpersonal skills.
• Proficiency in case management software and tools.
• Empathy, cultural competence, and ability to handle sensitive situations.
