Case Manager / Housing Facilitator

Openwork is hiring an experienced Case Manager / Housing Facilitator to support young adults (ages 18–24) as they work toward stable housing and independent living. This is a rewarding opportunity to make a meaningful impact by helping individuals access housing, community resources, and ongoing support.

• Assist clients with housing searches, lease applications, and move-ins.
• Help participants with housing vouchers and required documentation.
• Conduct assessments and develop individualized service plans.
• Provide ongoing case management and support.
• Maintain accurate case notes and client documentation.
• Connect clients with community resources and partner agencies.
• Build relationships with landlords and housing providers.
• Participate in program activities and client engagement efforts.

• 2+ years of case management experience required.
• Experience with housing programs, supportive housing, housing navigation, or housing vouchers required.
• Experience working with young adults or social service programs preferred.
• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Criminal Justice, or a related field required.
• Strong communication and organizational skills.
• Ability to manage multiple cases and maintain accurate documentation.
• Must be available to work evenings and weekends as scheduled.

• $26.00 per hour with weekly pay.
