Job Summary:

The Case Manager I is the primary responsible for the client journey from entry to exit. A Case Manager is responsible for the direct intervention in the lives of assigned clients which includes monitoring counseling, coaching, teaching, and helping clients become self-sufficient. Although assigned a specific case load, the Client Case Manager is also responsible for assisting all clients enrolled in the New Have program while on duty.

Job Responsibilities:

  • Consult with client on a weekly (more often as necessary) basis to establish goals, update progress, and discuss needs. Document all client interaction at the close of each case management session.
  • Action plans and goals should be discussed with clients and goals should be entered into the HMIS system for accountability.
  • Ensure accurate entry of all services provided, counseling notes, and other relevant data into the HMIS Database within 48 hours of the client interaction, and before the last day of the month to ensure reporting accuracy.
  • Participate in regular staffing meetings with other program staff.
  • Refer clients with specific needs to the appropriate counselor or program.
  • Enforce program guidelines and core values among all clients.
  • Conduct monthly Apartment inspections for the clients on the case load and record results in HMIS/Client Track.
  • Participate in trainings as assigned.
  • Plan and participate in client activities.
  • Assist in transportation of clients via passenger van, maintain authorized van driver status and complete all necessary driver trainings.

  • Participate in Ministry and prayer activities, as appropriate.

Qualifications:

Education,

Skills & Experience:

Requires a Bachelor’s Degree from an accredited college or university in social services‑related field (psychology, sociology, theology, religion, etc.) with 6 months but not less than 1 year of related work experience or an equivalent combination of education and experience. Valid Texas Driver’s License with acceptable driving record and current liability insurance. Ability to drive a 12‑15‑passenger van.

Good reading, writing, time management and ability to multitask. Good judgment in dealing with confidential client and donor matters and setting priorities. Capable of handling multiple tasks, coordinating detailed work and issues that need immediate attention. Ability to communicate and work cooperatively with employees, managers, directors, clients, volunteers and outside organizations that interact with Star of Hope.

Be able to work respectfully, professionally and in a Christ‑like manner with everyone, able to maintain required client/staff boundaries and possess a willingness to serve the Lord in a ministry that deals with homeless men, women and children.

Equipment Used:
General Office Supplies (Calculator, Telephone, Stapler, etc.), Computer Equipment (Monitor, Keyboard, etc.), Copier/Fax/Printer, Microsoft Word, Excel, PowerPoint, and Outlook. Fifteen-passenger van.

Work Environment:
Normal business hours, office environment, occasional weekend or evening events. Frequent visits with clients at program sites are required. Work schedule may be subject to change to meet the business needs of the mission. Exposure to infectious diseases, poor hygiene, drug usage, verbal and physical abuse and other emergency situations relative to the homeless population is normal. Business casual in accordance with Star of Hope dress code.

Relations with Others:
Constant contact with immediate peers and manager. Constant face to face discussions with individuals, electronic mail, written letters and memos. Frequent contact with peers in other departments and vendors. Frequent telephone conversation. Occasional contact with managers, clients, and volunteers.

Physical Requirements:

Light work with lifting up to 10 pounds. Constant sitting, use of fingers, talking, hearing and repetitive motions. Frequent standing, walking, and feeling. Occasional climbing, balancing, stooping, kneeling, crouching, reaching, lifting, handling and driving. Constant contact vision at 20 inches or less, and constant color vision and depth perception.
