Avenue 360 seeks an Outreach Navigator/Assessor to identify and connect individuals experiencing homelessness with housing resources through CES. The role conducts outreach, HMIS-based assessments, and pre-navigation services to secure required documentation for housing programs such as PSH and RRH.
Qualifications include a bachelor’s degree (or equivalent), two years in social services or related fields, and proficiency with HMIS and Microsoft Office. Valid driver’s license preferred.
