Job SummaryThe Care Coordinator manages and walks alongside residents during their life cycle within the Star of Hope Resident Journey. This includes ministry support, resident assessment, goal development, short- and long-term planning, one‑on‑one case management, accountability, conflict resolution, grievance support, discipline, termination processes when appropriate, and accurate documentation. The Care Coordinator ensures residents are connected to appropriate programming, services, classes, and resources that support their transition from homelessness toward healing, restoration, transformation, purpose, self‑sufficiency, and sustainable stability.
The position also supports facility order, program compliance, and collaboration across departments.
- Manage the resident life cycle from enrollment through progression, transition, or exit, while providing ministry support, goal planning, accountability, and support with grievances, discipline, or termination processes, when appropriate, providing Christ‑like care consistent with Star of Hope’s mission, core values, and faith statement.
- Assess resident needs, barriers, strengths, program fit, service needs, and next steps, and complete all related enrollments, exits, class assignments, services, and updates in Case Worthy.
- Meet with assigned residents weekly, or as directed, to develop, monitor, and update service plans, review progress, address barriers, establish goals, and support accountability.
- Connect residents to appropriate internal and external supports, including counseling, education, workforce development, medical care, benefits assistance, recovery resources, community agencies, and essential documents or services.
- Maintain accurate, timely resident documentation in Case Worthy and communicate key updates, concerns, progress, and risk factors to the assigned manager.
- Demonstrate effective communication and problem‑solving to support program structure, safety, and accountability by reinforcing rules, policies, attendance, program expectations, healthy conflict resolution, and appropriate resident conduct, including substance use screenings when assigned.
- Participate in meetings, trainings, care coordination, and collaboration with staff, volunteers, and leadership to support resident progress, stability, participation, and program completion.
- Support or teach program‑related classes, ministry activities, prayer opportunities, groups, workshops, and restorative programming from a Biblical perspective, and provide guidance to new Care Coordinator staff when assigned.
Job Responsibilities

Education,
Skills & Experience:
B.A. or B.S. in social services or other related field, with one year of related work experience; or an equivalent combination of education and experience. At least one year experience working with homeless or formerly homeless families/individuals preferred. Experience working with individuals with substance use and/or mental illness is helpful. Valid Texas driver’s license with acceptable driving record and current liability insurance.
Must be detail oriented, well organized and able to update and maintain accurate resident case files in a timely manner. Must have strong communication skills, both written and oral, and conflict resolution skills. Ability to speak in public and counsel individuals one‑on‑one. Proven problem‑solving ability and able to work in a team. Must be able to learn and retain information quickly in a fast‑paced environment.
Must have good judgement in dealing with confidential Star of Hope matters and resident information.
Be able to work respectfully, professionally and in a Christ‑like manner with everyone, able to maintain required resident/staff boundaries and possess a willingness to serve the Lord in a ministry that deals with homeless men, women and children.
