Licensed Social Worker - Director

Keystone, a subsidiary of the Ensign Group, seeks a licensed Social Worker to lead social services at Legend Oaks Healthcare and Rehabilitation Center – Northwest Houston. The position focuses on skilled nursing facilities serving post‑acute and long‑term care residents.

• Plan, develop, organize, implement, evaluate, and direct social service programs in accordance with federal, state, and local standards, as well as facility policies.
• Supervise staff such as Social Services Assistants and collaborate with leadership partners on policy and procedural changes.
• Maintain knowledge of current regulations and recommend updates to meet resident needs.
• Contribute to community and facility planning, including resident and family needs assessment.
• Participate in discharge planning and create individualized social care plans and resident assessments.
• Obtain social history and background information, providing compassionate support to families and residents.

• Bachelor’s degree in social work or a related human services field (e.g., sociology, special education, rehabilitation counseling, or psychology).
• Valid Texas license as a Social Worker.
• Minimum of one year of supervised social work experience in a skilled nursing or long‑term care setting.
• Strong interpersonal and communication skills with the ability to work calmly and effectively with residents, families, staff, visitors, and external agencies.
• Leadership ability and willingness to supervise and collaborate with others.
• Tact, patience, cheerful disposition, and enthusiasm for serving residents at all maturity levels.
• Openness to new approaches and willingness to integrate them into existing social services.
