Position Overview:
The Case Worker supports individuals and families in achieving long-term self-sufficiency by providing comprehensive case management, resource coordination, and personalized support. The Case Worker assesses client needs, develops individualized service plans, connects clients with community resources, and monitors progress toward educational, employment, financial, and personal stability goals
Key Responsibilities:- Conduct client intake, assessments, and eligibility screenings.
- Develop individualized self-sufficiency plans with measurable goals.
- Provide ongoing case management and supportive counseling.
- Connect clients to community resources, including housing, employment, education, healthcare, childcare, transportation, and financial assistance.
- Assist clients with job readiness, resume development, interview preparation, and employment referrals.
- Monitor client progress and update service plans as needed.
- Maintain accurate, timely, and confidential client records and case notes.
- Collaborate with community agencies, employers, and service providers to coordinate client services.
- Advocate for clients to help them overcome barriers to self-sufficiency.
- Conduct follow-up appointments to evaluate progress and ensure continued engagement.
- Prepare reports and maintain documentation in accordance with agency policies and funding requirements.
- Participate in staff meetings, training, and professional development activities.
- Preferably an associate or bachelor’s degree in social work, human services, psychology, counseling, or a related field. Alternatively, a community health worker with at least two years of casework experience is acceptable.
- Experience in case management, social services, workforce development, or community outreach preferred.
- Strong interpersonal, communication, and organizational skills.
- Knowledge of community resources and public assistance programs.
- Ability to maintain confidentiality and exercise sound professional judgment.
- Ability to work effectively with individuals from diverse cultural and socioeconomic backgrounds.
- Excellent problem-solving and critical-thinking skills.
- Ability to build trusting relationships with clients.
- Strong time management and documentation skills.
- Ability to manage multiple cases while meeting deadlines.
- Knowledge of trauma-informed and client-centered service approaches.
- Ability to work independently and as part of a multidisciplinary team
- Office-based with regular travel to community locations, partner agencies, and client appointments as needed.
- May require occasional evening or weekend hours to meet client needs.
- Clients successfully achieve individualized self-sufficiency goals.
- Increased participation in employment, education, or vocational training.
- Improved housing and financial stability outcomes.
- Timely and accurate case documentation.
- Compliance with program requirements and performance standards.
