Position: Part-Time Outreach / Case Manager

The Outreach / Case Manager will support Olive Branch Muslim Family Services by providing ongoing case management, client follow-up, vocational program support, community outreach, referral coordination, and accurate recordkeeping.

This position will work closely with the Client Advocate to ensure clients receive continuous support from initial intake through referral, service completion, and case closure. The Outreach / Case Manager will also help strengthen Olive Branch's relationships with community partners and identify additional resources for clients.

Duties CASE MANAGEMENT & CLIENT FOLLOW-UP
  • Follow up with clients referred by the Client Advocate.
  • Manage assigned client cases through completion.
  • Track referrals and confirm whether clients successfully received services.
  • Contact clients who require additional follow-up.
  • Assist clients with barriers preventing them from accessing services.
  • Maintain clear notes regarding client communication, referrals, outcomes, and next steps.
  • Continue follow-up until services are completed or no further Olive Branch action is required.
  • Document outcomes and close completed cases.
VOCATIONAL TRAINING SUPPORT
  • Assist clients applying for Olive Branch vocational training programs.
  • Review required documents for program eligibility.
  • Review financial eligibility documentation.
  • Follow up with clients regarding missing or incomplete documents.
  • Assist clients with completing enrollment requirements.
  • Coordinate with vocational training partners as needed.

  • Maintain accurate vocational program and waiting-list records.
OUTREACH & COMMUNITY PARTNERSHIPS
  • Conduct outreach to community organizations and service providers.
  • Develop and maintain relationships with nonprofit organizations, schools, healthcare providers, workforce programs, and other community partners.
  • Identify new referral resources based on client needs.
  • Follow up with partner organizations regarding available programs and services.
  • Assist with outreach for workshops, resource fairs, community events, and Olive Branch programs.
  • Conduct outreach for approved in-kind donations, including professional clothing, food, supplies, and other client needs.
  • Assist with partner and community engagement activities.
  • Confirm that organizations listed in the resource database are still providing services.
  • Update eligibility requirements, contact information, and program details.
  • Keep Olive Branch resource information current and organized.
  • Perform other duties and projects assigned by the supervisor.
DATA & RECORDKEEPING
  • Keep assigned client records and spreadsheets clean, accurate, and updated.
  • Enter and update client follow-up information in the master spreadsheet.
  • Track referrals, appointments, outcomes, and case closures.
  • Review records for missing information or overdue follow-ups.
  • Maintain organized electronic client files.
  • Assist with program statistics and reporting as needed.
Qualifications
  • Bachelor's degree in social work, human services, psychology, public health, nonprofit management, or a related field preferred.
  • Experience in case management, community outreach, client services, or nonprofit work preferred.
  • Strong organizational and follow-up skills.
  • Excellent attention to detail.
  • Strong written and verbal communication skills.
  • Ability to maintain accurate spreadsheets and electronic records.
  • Ability to independently manage assigned cases through completion.
  • Strong computer skills, including Microsoft Outlook, Word, Excel, SharePoint, and online research.
  • Ability to communicate professionally with clients, community partners, and service providers.
  • Ability to work effectively with diverse populations.
  • Bilingual or multilingual skills are highly valued.
