Overview
The LTC Charles S. Kettles VA Medical Center is seeking a Full-Time Licensed Practical Nurse for the Patient Aligned Care Teams (PACT) at the Howell VA Community Based Outpatient Clinic (CBOC) within Ambulatory Care Service. The tour for this position is:
Monday-Friday 7:30am-4:00pm.
- Citizenship:
Citizen of the United States - Education:
Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed - Verification can be obtained from the State approving agency or the NLNAC
- Additional information may be obtained from the NLNAC Website
- Exceptions include healthcare education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure
- Individuals granted a license by a jurisdiction that does not require graduation from an approved school are eligible
- Licensure:
Full active current and unrestricted licensure as a licensed practical or vocational nurse in a State/Territory/Commonwealth (e.g., Puerto Rico) or District of Columbia - English Language Proficiency:
Proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d) - Grade Determinations: GS-3 through GS-6 criteria summarized below; actual grade determined by experience and qualifications
- Total rewards of a Allied Health Professional; the LPN/LVN is a member of the larger team and requires general review of work by an RN for appropriateness
- Perform assigned nursing duties to promote quality health care and positive customer service
- Direct patient care; room patients; obtain vital signs; complete treatments as ordered by the primary care provider
- Phlebotomy, outpatient blood draws; clinic duties including toenail clinic, immunization clinic, quality metrics, population health metrics
- Follow-up on quality metrics fallouts (e.g., colorectal screening, breast/cervical cancer screens, immunizations)
- Administer medications as delegated; document actions; monitor patient response
- Assist with minor procedures, arrange necessary equipment, position patient, ensure comfort and privacy
- EKG, lab draws and pre-processing, lab quality monitoring, point-of-care testing
- Data collection and documentation of patient visit in nursing notes
- Provide health education to patients and families; support self-management, prevention, and wellness
- Assist in medication reconciliation and notify provider of changes; record vital signs in CPRS
- Apply skin tests; conduct vision tests and other routine assessments; monitor equipment and supplies for safety
- Schedule:
