Summary
Full‑time Licensed Practical Nurse for Patient Aligned Care Teams (PACT) at the Howell VA Community Based Outpatient Clinic (CBOC) within the Ambulatory Care Service of LTC Charles S. Kettles VA Medical Center. Tour:
Monday‑Friday 7:30am‑4:00pm.
- Citizenship:
United States citizen. - Education:
Graduate of a state‑approved practical or vocational nursing school, accredited by the National League for Nursing (NLNAC). Military training that satisfies licensing authorities is acceptable. - Licensure:
Full, active, unrestricted LPN/LVN license in any U.S. state, territory, Commonwealth, or District of Columbia. - Experience:
Depending on the grade level (GS‑3 to GS‑6), minimum experience ranges from none (GS‑3) to 1 year at the preceding level (GS‑5) or 1 year at GS‑5 for GS‑6. Refer to VHA Qualification Standards for exact criteria. - Proficiency in spoken and written English as required by federal regulations.
- Deliver direct patient care within the scope of an LPN/LVN, including medication administration, vital sign assessment, and basic nursing procedures.
- Perform phlebotomy, EKGs, laboratory draws, and other diagnostic assistance as directed.
- Document all care activities accurately in the electronic health record (CPRS). Monitor medication effects and report changes to supervising RN or MD/DO.
- Provide or supervise nursing assistants and other LPNs, ensuring adherence to VA policies and care standards.
- Participate in quality improvement initiatives, track screening metrics, and address fall‑out processes.
- Assist in emergency response, including cardiopulmonary arrest and equipment troubleshooting.
- Collaborate with interdisciplinary team members to offer patient and family education and health coaching.
- Prolonged standing and/or sitting for more than 4 hours.
- Ability to lift up to 45 pounds and perform repetitive stooping, bending, kneeling, squatting, carrying items, and patient transfers.
- Competitive salary; step increase may apply for higher qualifications.
- Paid time off: 37–50 days annually (13–26 days leave, 13 days sick, 11 federal holidays).
