Livingston County Community Mental Health seeks a Wraparound Facilitator to work with youth and families in homes, schools, and community settings. You will coordinate Child and Family Team meetings, develop family-driven Wraparound plans, and connect families with resources.
This role requires a Bachelor’s or Master’s degree in social work or related field, plus a valid MI driver’s license and willingness to work after-school/evening hours in Howell, Michigan.
