Overview

Join our primary care team as an Advanced Practice Nurse (APN) in a dynamic, outpatient-only setting. In this role you will lead patient-centered coordinated care, working collaboratively to assess, plan, and implement comprehensive patient care. The role demands strong competencies in clinical practice, leadership, and ethical decision-making.

Office hours:
Monday–Friday 8 a.m. – 8 p.m. and Saturdays 8 a.m. – 12 p.m. NP hours during the week can be 8 a.m.–4:30 p.m. or 12 p.m.–8 p.m.

Responsibilities
  • Perform and document accurate, complete, and relevant history and physical assessments appropriate for the patient’s age, gender, and clinical problem.
  • Order, perform, and interpret laboratory tests and diagnostic studies/procedures in collaboration with physicians and consultants as appropriate.
  • Manage general medical and surgical conditions based on comprehensive knowledge of etiologies, risk factors, pathophysiology, and indications/contraindications of pharmacologic agents and other treatment modalities.
  • Prescribe medication safely and effectively in accordance with best practices, hospital policy, and NJ regulations, providing patient education as needed.
  • Seek opportunities to promote Hackensack Meridian Health (HMH) APN practice recognition locally, nationally, and internationally through conferences, publications, and presentations.
  • Provide patient- and family-centered, compassionate care that promotes health and treats disease.
  • Practice as a health‑care provider according to the rules and regulations of the Medical Staff and bylaws outlined in the delineation of privileges.
  • Adhere to HMH nursing protocols, standards of care, and disease-specific quality indicators.
  • Apply evidence‑based and value‑based medicine to systematically analyze and improve patient care practices and outcomes.
  • Evaluate patient/family transition of care planning, considering resource utilization, cost, quality, safety, and self‑management promotion for chronic conditions.
  • Demonstrate clinical expertise through advanced assessment and procedural skills pertinent to the practice specialty.
  • Complete medication reconciliations as appropriate.
  • Participate in the annual APN peer review.
  • Translate best practice into clinical practice by evaluating current protocols and standards of care.
  • Provide culturally sensitive and educationally appropriate health‑management education to patients and families.
  • Facilitate consistent, coordinated care and clear communication among all members of the healthcare team through participation in daily interdisciplinary rounds.
  • Serve as an advocate for patients and families, identifying and addressing obstacles to care progression and escalating issues as appropriate.
  • Serve as a clinical resource for the multidisciplinary team by addressing clinical, process, and system improvement opportunities.
  • Participate in organizational and departmental meetings, committees, and work groups as necessary.
  • Demonstrate the highest level of accountability for professional practice and adhere to ethical principles regarding patient confidentiality, informed consent, and adverse outcomes.
  • Collaborate with medical staff to support nursing‑led protocols.
  • Mentor other APNs, precept students in APN programs, and participate in educational programs for nursing as a clinical expert.
  • Identify patient/family educational needs and collaborate to implement appropriate teaching materials.
  • Facilitate and participate in nursing educational programs as a clinical expert.
  • Promote a collaborative work environment supporting the organizational mission, behavioral expectations, values, and pillars.
  • Promote innovation by integrating new clinical knowledge into practice.
  • Participate in clinical research activities and disseminate findings through practice, education, presentations, and publications.
  • Identify ethical care issues and coordinate interdisciplinary meetings to resolve them.
  • Adhere to the legal framework and scope of practice defined by the NJ Division of Consumer Affairs, ensuring compliance with all applicable statutes and professional standards.
