Psychiatric Mental Health Nurse Practitioner Or Physician Assistant

We are seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant with psychiatric experience to join our expanding behavioral health team serving skilled nursing facilities throughout Columbia County, New York. This is a flexible part-time opportunity requiring approximately two to three days per week, making it an excellent fit for providers seeking supplemental income, those building a private practice, or clinicians looking for a long-term primary position with outstanding work-life balance.

Our providers become an integral part of the care team for residents who often have limited access to specialized psychiatric services. This is a mission-driven role where your clinical expertise directly improves the emotional health, behavioral stability, and quality of life of medically complex older adults living in skilled nursing environments. Providers with experience caring for patients with traumatic brain injuries (TBI) are highly valued, as many residents present with neuropsychiatric conditions requiring specialized assessment and treatment.

Requirements:

• Provide comprehensive psychiatric consultations, diagnostic evaluations, and medication management services within skilled nursing and rehabilitation facilities
• Evaluate adult and geriatric residents experiencing depression, anxiety, bipolar disorder, psychotic disorders, dementia-related behavioral symptoms, adjustment disorders, sleep disturbances, chronic psychiatric illness, and neurocognitive disorders
• Assess residents with traumatic brain injury and other neurological conditions contributing to psychiatric or behavioral symptoms
• Develop individualized treatment plans that address psychiatric, cognitive, behavioral, medical, and psychosocial needs
• Initiate, monitor, and adjust psychotropic medication regimens while considering age-related physiological changes and complex medical comorbidities
• Perform ongoing psychiatric follow-up visits to evaluate treatment response, symptom progression, medication effectiveness, and behavioral stability
• Collaborate closely with physicians, nursing leadership, therapists, rehabilitation specialists, social workers, psychologists, and interdisciplinary care teams
• Participate in care conferences and provide behavioral health recommendations designed to improve resident outcomes and quality of care
• Educate residents, caregivers, family members, and facility staff regarding psychiatric diagnoses, medication management, behavioral interventions, and treatment expectations
• Assist facilities in reducing psychiatric crises, behavioral escalations, avoidable hospitalizations, and emergency department utilization
• Maintain accurate, timely, and compliant documentation within electronic medical record systems
• Support evidence-based psychiatric care and participate in quality improvement initiatives across assigned facilities

Qualifications:

• Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) or Physician Assistant with significant psychiatry experience
• Active and unrestricted New York license
• Active DEA registration
• Strong background in psychiatric assessment, psychopharmacology, diagnosis, and treatment planning
• Ability to independently manage a psychiatric caseload while working collaboratively within interdisciplinary healthcare teams
• Experience working within skilled nursing facilities, rehabilitation centers, long-term care, behavioral health, neurology, or post-acute care preferred
• Experience evaluating and treating patients with traumatic brain injury (TBI) strongly preferred
• Experience managing dementia-related behaviors, chronic psychiatric illness, mood disorders, anxiety disorders, psychosis, and medically complex geriatric patients preferred
• Excellent communication, clinical reasoning, organizational, and relationship-building skills
• Passion for improving psychiatric care for aging and underserved populations
