As an Occupational Therapist, you will evaluate residents and develop individualized treatment plans that improve independence with activities of daily living, upper extremity function, cognition, and overall safety.
Responsibilities- Evaluate residents and establish individualized occupational therapy plans of care.
- Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
- Assess and recommend adaptive equipment and environmental modifications.
- Reassess patient progress and modify treatment plans as appropriate.
- Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
- Complete timely and accurate documentation using the electronic medical record (EMR).
- Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
- Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
- Current State Occupational Therapist (OT) License
- Current CPR/BLS Certification
- Valid driver's license, reliable transportation, and auto insurance
- Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
