The Clinical Care Coordinator works closely with the Registered Nurse (RN) to help oversee the clinical department and support the caregiving team. This role assists with daily clinical operations, staff scheduling, regulatory compliance, and coordination of resident care to ensure high-quality services for residents.
Key Responsibilities- Support the RN in overseeing daily clinical operations and the Clinical Care team
- Create and manage the clinical staff schedule and on‑call calendar
- Assist with onboarding and training of Clinical Care staff
- Monitor clinical documentation, physician orders, and updates in the electronic medical record
- Support medication ordering, cycle fills, and medication inventory
- Assist with care coordination including monthly weights, skin checks, and specialty appointments
- Communicate with families and care partners regarding resident needs and supplies
- Follow up on incidents and assist with clinical documentation compliance
- Conduct daily rounds to ensure a safe and well-maintained environment
- Assist with state surveys and ensure compliance with DHS 83 regulations and company policies
- High School Diploma or GED required
- 2+ years experience in assisted living or a similar healthcare setting
- Certified Nursing Assistant (CNA) preferred but not required
- CBRF certifications, CPR/AED certification, and Dementia/Alzheimer’s training preferred
- Strong organizational, communication, and leadership skills
