LPN-Program Director
E-VOLVE CARE Hudson, Wisconsin, United States
About this positionDescription:
- Oversee daily operations of two or more sites, ensuring consistent delivery of high-quality, person-centered services
- Ensure compliance with local, state, and federal regulations, including licensing requirements
- Lead and manage site supervisors and direct support staff; ensure adequate staffing levels and oversee recruitment and retention efforts
- Conduct regular supervision, coaching, and performance reviews of direct reports
- Maintain regular bi-weekly on-site presence across all assigned sites; establish a consistent routine of walk-throughs, audits, and check-ins
- Manage program budgets, monitor expenses, and ensure efficient use of resources in alignment with financial targets
- Support and monitor the development and implementation of individual service plans
- Foster positive relationships with guardians, case managers, and external stakeholders
- Ensure timely documentation, incident reporting, medication administration, and follow-up actions
- Collaborate with the Regional Executive Director on strategic and operational planning, program expansion, and talent development
- Support a culture of data-driven decision-making and staff accountability using KPI's
- Respond promptly to emergencies, staffing issues, or concerns from individuals or families
Shift: Monday to Thursday 8am to 4pm
Salary Range $55,000-$75,000
Requirements- (1) a bachelor's degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;
- (2) an associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;
- (3) a diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
- (4) a minimum of 50 hours of education and training related to human services and disabilities; and
- (5) four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3)
- (6) a minimum of three years of supervisory level experience in a program providing direct support services to persons with disabilities or persons age 65 and older
