As an RN Case Manager, you'll oversee client care from admission through ongoing services while providing skilled nursing visits in the home. You'll collaborate with physicians, caregivers, and families to ensure each client receives safe, personalized, high‑quality care.

This role is ideal for nurses who enjoy autonomy, relationship‑based care, and helping clients remain safely in their homes.

• Complete initial and ongoing nursing assessments
• Develop and update individualized care plans
• Provide skilled nursing visits and treatments
• Coordinate care with physicians, families, and caregivers
• Perform medication reconciliation and patient education
• Monitor changes in condition and communicate appropriately
• Promote client safety through home assessments
• Supervise and support caregivers as needed
• Maintain accurate electronic documentation
• Ensure compliance with state, federal, OSHA, and HIPAA regulations
• Participate in an occasional on‑call phone rotation

• Active RN license in Minnesota and Wisconsin
• Current CPR certification
• One or more years of nursing experience
• Home health or home care experience preferred
• Current TB test (or chest X‑ray if applicable)
• Valid driver's license with a clean driving record
• Strong assessment, organization, and communication skills
• Experience using electronic medical records
