As part of our in-house therapy model, you'll collaborate closely with nursing, leadership, and other rehab team members to make a direct, positive impact on resident lives—every day.

As our PRN Physical Therapist, you will play a key role in evaluating residents, developing personalized treatment plans, and delivering evidence-based interventions to support mobility, independence, and overall well-being. You'll work alongside a dedicated Interdisciplinary Team (IDT) and contribute to a therapeutic environment built on compassion, communication, and clinical integrity.

• Perform comprehensive evaluations of residents' physical conditions and functional abilities.
• Develop and implement customized treatment plans using evidence-based practices.
• Provide therapeutic interventions to improve mobility, balance, strength, and functional independence.
• Monitor progress and adjust care plans in collaboration with the IDT.
• Educate residents, caregivers, and families on mobility techniques and home exercises.
• Ensure timely and accurate documentation in line with facility and regulatory standards.
• Participate in team meetings and support an integrated care approach.
• Adhere to all safety, compliance, and regulatory requirements.

• Active Texas Physical Therapist (PT) license.
• Good standing with all relevant licensing boards/agencies.
• SNF or long-term-care experience preferred (not required).
• Familiarity with Net Health documentation system is a plus.
• Strong communication and interpersonal skills.
• Ability to work effectively with diverse adult and geriatric populations.
