Creative Solutions In Health Care is seeking a Per Diem (PRN) Physical Therapist to join our in-house rehab team serving residents in a SNF/long-term care environment in Texas. The PT will evaluate patients, develop and implement personalized rehabilitation plans, and document care in accordance with the plan of care and regulatory requirements.
The ideal candidate holds an active Texas PT license, has experience in SNF/LTC settings, and can collaborate with an interdisciplinary team.
