The Registered Nurse (RN) provides skilled nursing services and in‑home infusion therapy according to physician orders and individualized plans of care. The RN is responsible for assessing patient needs, administering infusion treatments, monitoring patient responses, educating patients and caregivers, and communicating changes in patient condition to the care team.
The ideal candidate will have strong vascular access skills and experience managing a variety of infusion therapies in the home setting.
ResponsibilitiesProvide skilled nursing care and infusion therapy services in patients' homes
Implement and manage individualized plans of care based on physician orders
Perform comprehensive nursing assessments and ongoing patient evaluations
Monitor patient response to treatments and promptly report significant changes in condition
Administer prescribed infusion therapies safely and effectively
Perform peripheral IV insertions and maintain vascular access devices
Provide care, maintenance, and infusion services through:
PICC lines
Central Venous Lines (CVL)
Midlines (ML)
Peripheral IVs
Port-a-Caths
Educate patients and family members regarding medications, infusion therapy, disease management, and treatment plans
Communicate effectively with physicians, pharmacists, patients, caregivers, and the Director of Nursing
Maintain accurate, detailed, and timely clinical documentation
Follow all agency policies, infection control procedures, and nursing practice standards
Treat patients, families, and team members with professionalism, courtesy, and respect
Current, unrestricted Texas Registered Nurse (RN) license
Current BLS/CPR Certification (in‑person certification required)
Negative TB skin test or chest X‑ray within the past year
Minimum one (1) year of documented nursing experience in a home health agency, primary care clinic, infusion setting, or healthcare facility required
Strong peripheral IV insertion skills required
Experience providing infusion therapy and managing vascular access devices preferred
Current and valid Texas Driver's License
Current automobile insurance
Reliable transportation
Strong assessment, communication, and documentation skills
Ability to stand, walk, bend, squat, kneel, reach, twist, push, and pull throughout the workday
Ability to safely assist patients as needed during care visits
Ability to independently lift up to 50 pounds
