Position: SOCIAL WORKER (BA/BSW)

Qualifications

Essential Core Functions:

Treatment Planning

Discharge/aftercare planning

Utilization review, including submission of continued stay requests

Chart reviews and medical record documentation, including reports

Communication with patients, families, physicians, staff and external entities

Conduct psycho-education and focus groups with patients

Patient/family education

Testifying at court and participating in MDT meetings. Transports youth.

Provides additional functions as needed by program.

Job Specifications:

Education, Certification/Licensure, and/or Experience

a. BSW in Social Work (preferred) or BA in Counseling or Psychology. MSW/MA preferred.

b. Social Work license in State of WV

c. One year experience with children preferred.

Knowledge, skills, and abilities required

a. Good verbal and written communication skills.

b. Knowledge of community resources

c. Knowledge of authorization of care, child welfare and juvenile justice processes

d. Ability to work collaboratively with others

e. Ability to assess patient/family needs for treatment and discharge planning purposes

f. Customer service

g. Treatment/discharge planning.
