The LPN provides direct care under the supervision of the RN Team Manager. The LPN is responsible for following the plan of care, providing treatment and working in conjunction with the members of the team in order to ensure comfort and quality of life.
Essential Functions- Provides routine nursing visits and personal care while following the plan of care:
- Monitors effectiveness of the plan of care in controlling pain and other symptoms
- Observes, collects, records and determines the need for more data
- Assists with nursing interventions to resolve current problems or prevent potential problems
- Report's findings to the RN team Manager and in the interdisciplinary meetings
- Provides comfort oriented physical care to patients:
- Provides wound care and changes dressings as needed
- Inserts and irrigates Foley catheter as ordered
- Administers enemas and removes impaction as ordered
- Tube feedings
- PCA
- Takes and records vital signs
- Assists with daily living activities
- Initiates change in patient's level of care as appropriate to meet the needs of the patient and/or family
- Provides continuous care nursing for the patient as directed in the patient's plan of care
- Provides and documents teaching to patients and/or families to assist in the patient's comfort and records the response of the family and/or family:
- Practices and role models universal precautions in all contacts with patients
- Demonstrates the physical care needed for the patients, family or Hospice Aide
- Provides emotional support to the patient and/or their families in a manner that fosters independence and affirms existing strengths
- Complies with the Privacy Policies of the organization to ensure compliance with the Health Insurance Portability and Accountability Act
- Represents and maintains the philosophy of the organization
- Performs other work duties as assigned

Knowledge and Skills- Understanding of the various regulatory guidelines (ethical, confidentiality, legal requirements, etc.)
- Demonstrates a working knowledge of the stages of uncomplicated grief and the dying process
- Possesses basic physical assessment skills
- Possesses the interpersonal skills that promote interdisciplinary collaboration and effective communication
- Minimum of 1-year patient care experience as a licensed practical nurse; hospice or home health preferred
- Valid driver's license and automobile insurance, and reliable transportation
Licensure in West Virginia and Ohio
Work Environment and/or Physical DemandsThe LPN sits, stands, bends, pushes and pulls, drives and lifts intermittently throughout the day. The LPN must have sufficient strength to turn, lift, and assist patients.

Additional RequirementsEmployment is contingent upon successful completion of a background check, drug screening, and possession of a valid driver's license.
