Geriatric Nurse Practitioner, Nurse Practitioner
Sub-Acute, Long-Term Care is a sector of healthcare exploding with potential and work-life balance flexibility (no on call or weekends). We are seeking a Full‑Time provider for our Heritage Center located in Huntington, WV. This position can be filled by a nurse practitioner or physician assistant, and new graduates are welcome to apply.
Base salary ranges from $100,000 to $116,000 per year, plus a no‑cap incentive compensation bonus program.

The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. The provider collaborates on‑site with the Center Leadership Team for center‑specific goals and priorities and works with the Medical Director and/or Attending Physician for patient‑specific medical care. The provider delivers primary and urgent care services to nursing center residents in collaboration with other medical providers, working collaboratively with Center Leadership to achieve Align Med Partners’ mission of improving lives through high‑quality health care and everyday compassion.
QualificationsAdvanced Practice Provider must be a graduate of an accredited School of Nursing program, licensed as a registered nurse, and certified as a Nurse Practitioner. Applicants should hold a Master’s Degree in Nursing with a concentration in Older Adult and/or Family Nurse Practitioner (FNP); other concentrations or specialty tracks may be considered based on experience and business need. Physician Assistants must have graduated from a program accredited by the ARC‑PA, passed the PA National Certifying Exam (PANCE) to use the PAC designation, hold state licensure, and possess NCCPA certification.
A minimum of two years of experience working with adult or geriatric populations is preferred. In states where practice allows, APRNs must hold an unrestricted professional license and DEA for each state in which they practice; PAs must meet the same licensing requirements.

Restrictions apply based on collective bargaining agreements, applicable state law, and factors such as pay classification, job grade, location, and length of service. Eligibility for full‑time benefits requires a minimum of 30 hours per week. Certain benefits, including health plans, are not available to part‑time employees working between 16 and 29 hours per week.
