Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Huntley, Illinois. The position is full‑time (40 hours per week) with an 8‑hour shift in a staff employment type. This role supports patients in Palatine, Huntley, Crystal Lake and surrounding areas and is eligible for a $10,000 first‑year success bonus to reward key achievements during the first year.
ResponsibilitiesOur home health registered nurse case managers lead and collaborate to provide management and delivery of patient care plans.
Qualifications- RN license in the state you work.
- Graduate from an approved school of practical nursing.
- One year of RN experience and the clinical competence to deliver quality patient care.
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
- A commitment to consistently meet critical deadlines for charting.
- The skills needed to self‑manage your time and schedule.
- Demonstrated experience with tablets, mobile phones and EMR software.
Successful completion of a drug screen prior to employment is part of our background process, which includes medical and recreational marijuana.
