Registered Nurse, Home Health

• Ensure all activities align with Elara Caring's vision and Home Health leadership
• Maintain patient and staff privacy and confidentiality under HIPAA Privacy Final Rule
• Assess assigned home health patient and family caseloads for physical, psychosocial, and environmental needs
• Assume primary responsibility for caseloads, including assessing, planning, coordinating, implementing, and evaluating treatment plans
• Collect physical, psychological, social, and spiritual patient data and incorporate it into plans of care
• Perform initial evaluations and re-evaluate nursing needs during each visit
• Make follow-up visits according to patient, family, and facility needs
• Communicate significant findings, problems, health changes, environmental issues, and unsafe facility conditions to relevant clinical and care personnel
• Revise plans of care in response to patient care issues and notify the Clinical Supervisor, physician, facility, and team members

• Current, unrestricted state RN license
• At least 1 year of experience in a clinical care setting as a nurse
• Ability and willingness to travel within the branch/office coverage area
• Dependable vehicle, valid driver's license, and current auto insurance meeting state laws
• Ability and willingness to travel 50%
• Ability to sit, stand, bend, lift, and move intermittently
• Ability to lift 50-100 lbs.
• Associate Degree or Bachelor's Degree in Nursing preferred
• Previous Home Health or Hospice experience preferred
• Previous Home Care Home Base experience preferred
• Compliance with HIPAA privacy and confidentiality requirements
• E-Verify/Form I-9 employment verification after offer acceptance

Demonstrates expertise in patient assessment, care planning, and coordination within home health settings, ensuring compliance with HIPAA regulations and effective communication with clinical teams. Holds a current RN license and possesses the ability to manage diverse patient needs while maintaining confidentiality and privacy.
