Elara Caring invites experienced Speech‑Language Pathologist to join our team and provide high‑quality home‑care services to over 60,000 patients daily.

• Promote Elara Caring’s philosophy, mission, and administrative policies to ensure quality of care.
• Evaluate patients to determine rehabilitation needs and potential, and develop a care plan in accordance with findings.
• Admit patients and assess speech‑language therapy and all other needs following OASIS and PPS regulatory guidelines.
• Conduct initial therapy evaluation visits and re‑evaluate patients during each visit.
• Perform follow‑up assessments according to OASIS and PPS guidelines.
• Initiate plans for patient safety, utilizing patient, family, and community resources.
• Communicate significant findings, problems, and changes to supervisors, physicians, and team members.
• Participate in implementation and development of the Plan of Care to ensure quality and continuity of care and proper discharge planning.
• Verify the Plan of Care before each visit and provide care per physician orders, assessment data, and established standards.
• Write physician orders to cover additional visits and changes to the Plan of Care.
• Incorporate patient care goals from the Plan of Care into therapy care, evidenced by documentation.
• Initiate and revise the Plan of Care in response to identified patient care issues.
• Perform direct speech‑language pathology services per accepted standards of practice and physician certification.
• Select diagnostic and therapeutic materials appropriate to patient needs.
• Instruct patients or caregivers on home therapy programs, providing modifications and clarification.
• Ensure clinical notes reflect ongoing communication and coordination of services with physicians and staff.
• Maintain accurate therapy notes, supply logs, and visit logs per office policy.

• Master’s degree from an accredited educational program in Language Pathology.
• Current, unrestricted SLP license in the state of practice.
• Minimum one year of experience as an SLP in a clinical care setting.
• Dependable vehicle, valid driver’s license, and current auto insurance for travel within coverage area.
• Willingness and capability to travel 50% of time, including travel within branch/office coverage area.
• Ability to sit, stand, bend, lift, and move patients intermittently; capable of lifting 50–100 lbs.

Base salary range: $81,000 – $111,000 annually. Salary may include additional earnings for work beyond quota, bonuses, on‑call, or other performance‑based compensation variables. Benefits include:
