Nurse Practitioner; PRN - In-Home Health Assessments

Our national network of clinicians meets members wherever they call home, helping older adults and other at‑risk individuals enjoy a better quality of care, improved health outcomes and the identification of chronic conditions that may otherwise go undiagnosed.

• Conduct adult or geriatric comprehensive health assessments, including medical history, diagnosis, treatment, health education, physician and case‑management referrals.
• Execute point‑of‑care and laboratory testing as needed and clearly document findings according to Matrix guidelines.
• Collaborate with physicians, case managers, social workers, family members, key caregivers and ancillary medical personnel.
• Partner with the Primary Care Physician on patient education and follow‑up.
• Provide services at the member’s place of residence, including homes and skilled nursing facilities.

• Flexibility
  :
  Visits are scheduled based on your availability, from Sunday to Saturday, 8 am–7 pm.
• Competitive Pay
  :
  Income is based on the number of assessments completed; 20–100 visits per month can earn $1,800–$11,500 monthly, based on a per‑assessment rate of $90–$115.

• Full‑time Opportunities
  : PRN providers can transition to full‑time roles in their home location, state or region and receive a full‑time sign‑on bonus.

• Master’s Degree or equivalent experience and successful completion of NP licensure.
• Current NP licensure in the state of practice, including prescription authority or the ability to obtain it.
• Board certification from AANP, AACN or ANCC in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency or Acute Care specialty.
• Current BLS, ACLS or CPR certification.
• One year of NP experience preferred; new graduates are encouraged to apply.
• Valid state driver’s license and automobile insurance as required for the state of residence.
• Insurance coverage:
  Bodily injury $100,000 per person/$300,000 per incident;
  Property damage $50,000;
  Collision and comprehensive deductibles $1,000 or less.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.
