Job Summary

The Nursing Quality Assurance Coordinator is responsible for the overall quality assurance and performance improvement program for the agency/branch, working with nursing and pharmacy teams. Responsibilities include patient teaching for new nursing patients, medical record audits, data collection and analysis, compliance with state regulations, accreditation preparation, educational activities, and maintaining documentation of quality improvement activities.

Major Duties And Responsibilities
  • Perform medical record audits and review nursing documentation to support quality improvement.
  • Collect, analyze and report data for agency programs and services.
  • Assist with assessment of compliance with state regulations, licensing status and ACHC accreditation.
  • Support preparation for accreditation and certification surveys, involving staff at all levels.
  • Coordinate educational activities related to the quality assurance/performance improvement program for all employees.
  • Lead quality improvement/PI teams and maintain documentation of all team activity.
  • Serve as a resource for staff regarding quality improvement and documentation.
  • Orient new employees on documentation and improvement processes.
  • Ensure timely completion of all agency quality improvement activities.
  • Coordinate quarterly reporting functions and write report summaries.
  • Create and manage Plan of Treatment/Plan of Care documents, track recertifications, and monitor documentation submissions by nursing staff.
  • Collaborate with the Intake team to track annual consent logs and follow up with nursing staff and patients.
  • Maintain patient census and discharge records in the Computer System.
  • Perform chart audits for active and discharged patients each quarter.
  • Provide virtual or in‑person initial patient education for new nursing patients at hospital or patient home.
  • Assist with nursing visits to patients in home as needed, including branch nursing on‑call rotation and supervisory visits for nursing competency.
  • Other duties as assigned.

Education And/Or Experience
  • Registered nurse or licensed vocational nurse with an active license in the state of practice.
  • Minimum two years of nursing experience.
  • Minimum two years of experience in clinical record audits, care plan development, or QI audit functions.
  • Current CPR/BLS certification.
  • Valid driver’s license and a reliable, insured vehicle in good working condition.
Knowledge And Abilities
  • Knowledge and skills to provide care and communicate with patients.
  • Ability to assess data reflecting client status and interpret relevant information to identify client needs.
