The Weekend Baylor RN provides and directs nursing care to patients in their homes as prescribed by physicians, ensuring compliance with applicable laws and agency policies. • Location: Huntsville, AL
• Facility: Huntsville Hospital Home Care
• Brand/Division: Optum
• Schedule: Full Time, Weekend Baylor
• Pay: $74,560 - $111,840 annually

• Develop, implement, and update patient plans of care in collaboration with physicians and the interdisciplinary team.
• Ensure home-health eligibility and complete comprehensive nursing assessments (including OASIS and medication reconciliation).
• Provide and coordinate high-quality clinical care; supervise and assign tasks to home health aides and LPN/LVNs.
• Accurately document all patient care per policy and payer requirements.
• Communicate timely with physicians, patients, families, and agency staff to ensure coordinated, effective care.
• Participate in orientation and precepting of new staff.
• Contribute to performance improvement, survey readiness, patient safety initiatives, and reducing avoidable hospitalizations.
• Demonstrate professionalism through punctuality, positive attitude, and adherence to agency standards.
• Function in any home situation regardless of client demographics.

• Education:
  • Current and unrestricted RN licensure in the state of Alabama
• Experience:
  • 1+ years of clinical experience preferred
