Optimal Care - Evergreen is seeking a licensed Physical Therapist to join our clinician-owned home health team. You will provide one-on-one rehabilitation in patients' homes, helping them regain strength, improve mobility, and return to daily activities.
You'll assess functional abilities, develop individualized treatment plans, and collaborate with an interdisciplinary care team to educate patients and caregivers, while traveling daily within the Jackson, MI service area.
