Licensed Practical Nurse

Putnam Care Center in Milford, CT is seeking a compassionate Licensed Practical Nurse to deliver high-quality care in our skilled nursing and rehabilitation facility. The LPN will provide direct bedside care, administer medications, monitor resident conditions, and support individualized rehab and long-term care plans. Responsibilities include documenting in EMR, assisting with ADLs, wound care, and safety initiatives, and collaborating with RNs, therapists, and physicians.

Responsibilities

• Provide direct bedside care to residents in a skilled nursing and rehab setting
• Administer medications and treatments per physician orders and facility protocols
• Monitor and document vital signs, resident status, and responses to care in EMRAssist with wound care, mobility, ADLs, and rehab-focused care plans
• Collaborate with RNs, therapists, and physicians to update care plans
• Communicate changes in resident condition promptly to the care team and families
• Maintain infection control, safety, and fall prevention standards
• Precept and support CNAs and new staff to ensure quality resident care
