Registered Nurse; RN; Home Health Case Management

We are seeking a compassionate Registered Nurse (RN) Home Health Case Manager to provide patient-centered care in Hutchinson, Kansas, and surrounding areas.

• Responsibilities include:
  
  managing a personal caseload, supervising LPN visits, conducting routine care, evaluations, and starts of care, educating patients on health conditions such as CHF, COPD, Diabetes, Wound Care, and Medication Management, collaborating with physicians to develop diagnosis-driven plans, and working with a multidisciplinary therapy team including PT, OT, and Speech.
• Qualifications:
  
  active Kansas or Compact RN license, valid Missouri Driver's License, strong clinical assessment skills, organized, self-directed, and compassionate communicator.
• Key skills: effective patient and family communication, regulatory compliance, and ability to manage own schedule.
