Our BRIDGES to Success division is looking to add a program manager to our Litchfield team. The ideal candidate would be outgoing, organized, compassionate, a great problem solver, and enjoys creating activities and working with both employees and clients.

BRIDGES to Success provides short and long-term support and services to adults with physical and mental disabilities who are living at home, with a family member, or out in the community. BRIDGES stands for Building Relationships and Independence through Determination, Guidance and Educated Supports.

Location
  • Cokato, MN
Hours Of Position
  • Full-time
  • Flexible based on client schedule
  • Typically day-time business hours are 8am-4pm
  • Some on-call
Compensation
  • $24/hour
Typical Duties
  • Manages own caseload of clients and works one-on-one providing a variety of services
  • Provides supervision and oversight for the caseloads of the staff they supervise
  • Ensures compliance with licensing requirements
  • Coordinates staff training/meetings
  • Participates in leadership meetings
  • Assists with developing and providing services that improve independent living
  • Analyze and provide monthly reports
Requirements
  • A bachelor's degree in a field related to human services
  • OR 4 years of full-time experience working with individuals with challenging behaviors as well as co-occurring mental disorders or neurocognitive disorder
  • Organization, communication, time management, and supervisory skills are essential for success in this position
  • 2+ years experience in a supervisory role is preferred
  • Prior experience with 245D licensing and person-centered planning and delivery
  • A valid driver's license, clean MVR & auto insurance (You will be using your personal vehicle to transport clients)
  • A working mobile device that can receive & send phone calls and capability of downloading application for documentation
  • Compassionate and caring demeanor
  • Ability to work both independently and with a team
  • Effective organization, time management, and ability to manage time and meet deadlines is necessary
