We are seeking a dedicated Nurse Practitioner or Physician Assistant to join our growing Post‑Acute Care team Cape Cod, MA. This full‑time position provides an excellent opportunity to deliver high‑quality, patient‑centered care to individuals transitioning from acute hospital settings to skilled nursing and rehabilitation facilities.

Additional Openings:
Wakefield, Rhode Island and North Conway, New Hampshire.
Providers will work collaboratively with interdisciplinary teams to manage chronic and acute medical conditions, support recovery, reduce hospital readmissions, and optimize patient outcomes in the post‑acute setting.

Position Highlights:
  • Full‑time schedule:
    Monday – Friday, 9:00 AM – 5:00 PM
  • No weekends
  • No call responsibilities
  • Facility‑based rounding in post‑acute care settings
  • Autonomy with strong clinical and operational support
  • Opportunity to build long‑term relationships with patients and care teams
Compensation & Benefits:
  • Base salary: $120,000 annually
  • Total earning potential: $180,000 – $220,000+

Requirements:
  • Graduate of an accredited Nurse Practitioner or Physician Assistant program
  • Current and unrestricted NP or PA license in the applicable state or eligibility for licensure
  • National board certification as a Nurse Practitioner or Physician Assistant
  • DEA registration or eligibility to obtain
  • Minimum of one year of clinical experience preferred; experience in post‑acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long‑term care strongly preferred
  • Strong clinical assessment, diagnostic, and care coordination skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently while collaborating effectively within a multidisciplinary team
  • Proficiency with EMR systems
