Job Title

RN Case Manager MNA, VNA of Cape Cod

Purpose of Position

As a vital member of the interdisciplinary team, the Hospice RN provides skilled nursing care, supportive care, and pain and symptom management to maximize comfort and quality of life for hospice patients and their families. The RN provides care to patients in both home and facility settings.

Responsibilities
  • Participates in interdisciplinary patient case conferences as needed.
  • Utilizes critical thinking to assess patients and respond appropriately to critical changes.
  • Manages time efficiently and prioritizes differing patient needs and stages of health.
  • Demonstrates excellent verbal and written communication skills.
  • Interacts efficiently and compassionately with patients, families, co‑workers, and management.
  • Provides patient care with a compassionate and caring attitude.
  • Notifies MD or other involved care providers of significant changes in the patient’s condition.
  • Completes admission and other paperwork in a timely manner.
  • Records narrative documentation to reflect skilled care; documentation is clear, concise, and addresses teaching, direct care, and patient/caregiver response.
  • Obtains and clarifies valid MD orders for all care delivered.
  • Ensures completion of admission paperwork by 9 am of the next business day at the latest.
  • Adheres to nursing standards set by the American Nurses Association and VNA Hospice policies and practices.
  • Explains the philosophy, purpose, scope, and role of hospice care to patients/families and nursing home staff and is able to assess and admit patients as needed.
  • Implements the Plan of Care under the attending physician’s direction, ensuring compliance with federal and state regulations.
  • Supervises hospice aides every 14 days per regulations.
  • Assists in orientation of new staff by acting as a mentor when requested.
  • Assumes additional responsibilities as assigned by the manager.
Qualifications
  • Successful passage of the VNA pre‑hire medication test.
  • Current RN license in Massachusetts and related credentials as required.
  • One (1) year of nursing experience required, or six (6) months of nursing experience with a BSN.
  • Hospice home‑care experience, IV certification, computer skills, and a BSN preferred.

Pay range displayed reflects the anticipated range for new hires. Compensation will be determined after reviewing work history, experience, skill set, and education. Benefits include healthcare, dental, vision, and retirement.
