Continuum Hospice is hiring a Full Time Compassionate Resource Nurse (RN) to provide patient care in and around the Cape Cod Area. The RN will work with the intake team and referral sources to promote hospice care, including pre-assessments, information sharing and admissions in patients' homes or healthcare facilities.
