Job Summary

The Nurse Practitioner is a registered professional nurse with advanced graduate education and clinical training who provides comprehensive services in compliance with the state Nurse Practice Act. The role includes hospice care through evaluation and assessment of health history, health promotion, diagnosing medical conditions, and management of health problems by developing care plans, prescribing medications, and coordinating with patients, families, and other healthcare practitioners.

Job Requirements
  • Graduation from an accredited School of Nursing and an accredited Nurse Practitioner Program.
  • Current nursing licensure in the state and CPR certification.
  • Master’s degree with at least one year of Nurse Practitioner experience and at least one year of hospice and palliative care experience.
  • Accreditation from an approved certifying body for advanced practice nursing as required by state regulations.
  • Preferred specialty certification (e.g., Hospice and Palliative Nursing (CHPN), Pediatric, Adult, or Geriatrics).
  • Excellent observational, verbal and written communication, problem‑solving, and mathematical skills; nursing skills per competency checklist.
  • Ability to walk or stand for prolonged periods, lift and transfer patients and supplies, and reach, stoop, bend, kneel, or crouch as needed.
  • Adequate visual acuity and hearing for required nursing skills.
  • Licensed driver with an insured automobile in good working order in accordance with state or organization requirements.
Job Duties
  • Provide medical care, palliate, and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician.
  • Complete history and physical examinations, evaluate the patient's medical condition and health history, order and interpret laboratory and diagnostic results, diagnose health conditions, and document findings in the medical record.
  • Develop individualized care plans, prescribe medications or treatments, obtain consultations or referrals, and coordinate hospice care services.
  • Complete hospice face‑to‑face visit requirements before the third and subsequent benefit periods to assess eligibility based on hospice participation conditions and local coverage determinations.
  • Provide clinical information to the hospice medical director for eligibility and recertification decisions.
  • Make recommendations regarding patient eligibility for certification and recertification for hospice care.
  • Review clinical information, coordinate, and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners.
  • Ensure patients receive appropriate symptom‑control measures through collaboration with interdisciplinary team members.
  • Review and develop treatment protocols and propose intervention options based on literature in collaboration with the collaborating physician.
  • Consult the collaborating physician when the patient's plan of care falls outside standardized practice and protocols.
  • Communicate and collaborate with the interdisciplinary group to create, review, and revise patient care plans.
  • Consult with the patient’s attending physician, the hospice medical director, and other healthcare practitioners regarding eligibility for hospice care and ongoing medical management.
  • Communicate with other community health practitioners to coordinate the plan of care.
  • Attend and participate in hospice interdisciplinary group meetings.
  • Complete, maintain, and submit accurate and relevant clinical notes and documentation in the medical record.
  • Educate/counsel patients, families, and caregivers about preventative care, medical problems, psychological issues, and spiritual concerns in conjunction with the interdisciplinary team to meet total patient needs.
  • Provide and maintain a safe environment for the patient.
  • Assist the patient, family/caregiver, and team members in providing continuity of care.
  • Work in cooperation with the family/caregiver and hospice interdisciplinary group to identify goals of care and meet the care needs of patients and families.
