This flexible position comes with a full-time medical assistant and is instrumental in providing competent, cost-effective, timely, and high-quality health care services while helping patients and their families navigate the complex medical system.
This role will represent Symbii Provider Services while being an employee of Symbii Home Health and Hospice.
- Under the supervision of the medical director, effectively deliver clinical and associated operational responsibilities.
- Maintain performance and productivity standards, operational efficiency, excellent customer service and best practice standards.
- Treat acute and routine medical conditions via in person, telephone call or telehealth across all settings (e.g. SNF, ALF and house call visits).
- Demonstrate critical thinking, expert care, and best practice standards in the clinical decision‑making process.
- Provide care for patients from initial history taking, clinical assessment, diagnosis, prescribing medications and treatments, providing education and evaluation of care.
- Maintain compliance with the established standards of practice of medicine for Nurse Practitioner in the State of Idaho.
- Document findings and actions appropriately in the patient’s medical record.
- Maintain patient confidentiality at all times.
- Participate in professional development activities and maintain professional affiliations and CME’s.
- Maintain safe work environment for self, patient, family, and staff by using appropriate PPE.
- Work collaboratively with Symbii Home Health and Hospice team members and community partners such as ALF and SNF staff members, patient family members and home health and hospice team members.
- Represent Symbii Home Health and Hospice in a professional manner and strive to promote a cooperative atmosphere at all times by acting with tact and diplomacy and maintaining a positive attitude.
- Maintain professionalism at all times by being dependable, autonomous, and responsible.
- Prioritize workload and utilize work time properly and productively.
- Completes the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility. Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification and makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.
- Reviews clinical information, coordinates and oversees patient care in consultation with the hospice interdisciplinary team.

Job Requirements- Master’s Degree from an accredited NP program, possess appropriate state licensure, prescriptive authorization, APN authorization, and meet credentialing requirements.
- Must be able to pass a background check and drug test.
- Must maintain a current and valid driver’s license and auto insurance and have access to reliable transportation.
- Must be able to read, write, and speak English fluently.
- Ability to communicate effectively, in verbal and written form, with medical partners at various levels, patients, family members, physicians, and representatives of the community.
- Maintain current CPR certification.
