As the Registered Nurse Case Manager, you will make decisions for your patients’ health and collaborate with the physician, family, and other members of the care team. - Be a part of a large caregiving community

You will have the opportunity to develop relationships with other caregivers in the facilities and physician’s offices of your patients as you coordinate care.
- Develop and coordinate the plan of care with the interdisciplinary team and provide proper preventative and rehabilitative nursing care as ordered by the physician and as indicated in the plan of care.
- Provide the patient and their caregivers counseling and education to facilitate overall wellness.
- Communicate with the physician, your interdisciplinary team, and other caregivers to coordinate the best care possible for your patients.
- Prepare clinical notes and identify discharge planning needs as part of the care plan development and implement prior to discharge of patient.
- Instruct, supervise, and evaluate CNAs and LPNs, as directed.
- Must possess a valid RN license in the State of Idaho
- Must hold a current CPR certification
- Must hold a valid driver’s license and current auto insurance
