Licensed Practical Nurse

Life Care Centers is seeking an LPN Licensed Practical Nurse to join our Healthcare & Medical Services team. Responsibilities

• Provide compassionate, hands-on nursing care to senior residents in a long-term care and rehabilitation setting, following individualized care plans and physician orders.
• Administer medications and treatments safely and accurately, including oral, topical, and injectable medications, and monitor residents for therapeutic and adverse responses.
• Perform routine nursing procedures such as vital sign monitoring, wound and skin care, catheter care, and basic respiratory care within LPN scope of practice.
• Support rehabilitation goals by assisting residents with mobility, exercises, and activities of daily living to promote independence and optimal functional outcomes.
• Accurately document all care, observations, and resident responses in the Electronic Health Record (EHR) in a timely and compliant manner.
• Communicate effectively with RNs, physicians, therapy staff, CNAs, and other interdisciplinary team members to coordinate comprehensive, patient-first care.
• Observe, recognize, and promptly report changes in resident condition, and participate in implementing appropriate interventions.
• Educate residents and families about medications, treatments, and self-care strategies, providing emotional support in a family-focused environment.
• Help maintain a safe, clean, and dignified environment for residents, adhering to infection control, safety, and quality standards.
• Participate in shift handoffs, care conferences, in-service trainings, and ongoing professional development activities to support clinical growth and high-quality outcomes.
