Care Manager

Morning Star Senior Management LLC is seeking a Care Manager for our Assisted Living community. In this role, you'll coordinate resident care plans, oversee daily health and wellness needs, and guide caregivers to deliver compassionate, person-centered support. You'll collaborate with families, physicians, and community partners to ensure safety, dignity, and quality of life for each resident. Working in our faith-inspired, servant-leadership culture, you'll model hospitality, integrity, and respect while mentoring team members and supporting regulatory compliance and documentation.

Responsibilities

• Develop and update individualized care plans for assisted living residents
• Monitor resident health, behavior, and functional status and respond to changes
• Oversee caregivers and medication aides to ensure high-quality, person-centered care
• Coordinate with physicians, nurses, and outside providers for resident services
• Communicate regularly with residents' families and address concerns promptly
• Ensure compliance with state regulations, company policies, and care standards
• Document assessments, incidents, and care interventions accurately and timely
• Support onboarding, training, and mentoring of caregiving staff
• Participate in care conferences and interdisciplinary team meetings
• Promote a safe, respectful, and faith-inspired community environment
