Discovery Rehabilitation in Salmon, , is a post‑acute care facility with long‑term, short‑term and geriatric services. We offer a stable, fun in‑house therapy team and excellent programs to support our geriatric clientele on their journey to recovery. We are currently seeking a full‑time Occupational Therapist for our in‑house rehab program.
Duties- Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate plan of care following all regulatory and clinical practice standards.
- Utilize standardized assessments and evidence‑based practice to support clinical interventions.
- Provide rehabilitative, skilled and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
- Administer a variety of procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc.
- Assess patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
- Evaluate treatment outcomes for effectiveness; modify services based on evaluation and refer patients as appropriate.
- Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
- Effectively communicate with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
- Provide comprehensive discharge summaries of services per regulatory and clinical requirements.
- Licensed Occupational Therapist.
