Requirements

The Med‑Surg LPN provides safe, compassionate, and efficient nursing care under the direction of a Registered Nurse: adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; participating in the development and modification of care; following facility standards of care set by the State Board of Nursing and other governing agency regulations; and possessing knowledge and skills regarding nursing principals and practice.

The LPN is responsible for nursing care assigned to that position and directs others in patient care. Demonstrates the ability to deliver care based on patient needs appropriate to age and developmental level. Responsible for the daily organization of unit, patient care, and other nursing personnel with less experience. You should possess a basic working knowledge of nursing principles. Decision making limited by hospital policy and procedures.

This position will demonstrate competency in the care of the:
Neonate, Infant, Child, Adolescent, Adult, and Geriatric patient population(s).

Summary

Essential Duties and Responsibilities
  • Provides direct nursing care under the supervision of an RN, including assessments within LPN scope, vital signs, and focused observations.
  • Incorporate the nursing process to assess, plan, and evaluate the patient.
  • Prioritize patient care based on assessment of patient health care status.
  • Collaborate with other health care providers in the provision of patient care.
  • Report significant and accurate data regarding aspects of care to the appropriate people.
  • Under the supervision of an RN, revise plan of care based on patient outcomes.
  • Teach, advise, direct/inform others under the supervision of an RN.
  • Possess knowledge of pharmacology. Adhere to pharmacy policies and procedures.
  • Maintain accurate, timely documentation in the electronic health record.
  • Participate in admission, transfer, and discharge process.
  • Assist with fall prevention, infection control, and other safety initiatives.
  • Report/record information legibly and appropriately.
  • Communicate effectively with a wide range of age groups, ethnic, educational, and socioeconomic backgrounds.
  • Make nursing decisions within the scope of licensure and according to established hospital policies and procedures.
  • Attend 75% of nursing staff meetings and other available in‑service education that pertain to professional growth and development.
  • Attend mandatory in‑services.

  • Able to work weekends, holidays, and shift rotations as needed.
  • Potential risks of exposure to hazardous materials, communicable diseases, radiation, blood/body fluids.
  • Considered critical services during inclement weather and disasters.
  • Maintain good physical, emotional, and mental health.
  • Be a patient advocate.
  • Observation to all patients for skin breakdown and assess for potential bed sores.
Position Specific Competencies
  • Uses the nursing process to assess, plan, and evaluate patient care in collaboration with the RN.
  • Provides nursing care, in collaboration with the RN, with consideration of the physical, psychosocial, cultural/spiritual, and age specific needs of those served.
  • Follow hospital policies, nursing standards, and regulatory requirements.
  • Maintain medication safety, infection prevention, and documentation accuracy.
  • Participate in quality improvement activities and unit‑based initiatives.
  • Demonstrate appropriate use of equipment/supplies.
  • Use appropriate safety and infection control measures. (falls prevention and infection prevention)
  • Teach, direct, advise and inform others in an appropriate manner under the supervision of an RN.
  • Report and records information legibly and appropriately.
