Home and Community Based Services Licensed Practical Nurse
Provide direct nursing care in patients' homes for acutely or chronically ill patients, record clinical and progress notes, and coordinate patient care activities to deliver high quality patient care in the geographic service area, under the supervision of a Registered Nurse or designated supervisor.
Primary Responsibilities- Provides high quality clinical services within state nursing scope of practice, in accordance with the plan of care, and in coordination with other members of the health care team, following all infection control procedures.
- Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record, using the agency's electronic visit verification program (as applicable).
- Actively participates in the agency's care coordination process, including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with the patient, family, physician, other clinicians, agency management staff, and case coordinator or community case managers (as applicable).
- Current and unrestricted Licensed Practical Nurse licensure in the state of practice.
- Current CPR certification.
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
- Ability to function in any home situation regardless of client age, race, creed, color, sex, disability, or financial condition.
- Home care experience.
- Ability to work flexible hours to meet client needs.
- Ability to manage multiple tasks simultaneously.
- Able to work independently.
- Good communication, writing, and organizational skills.
Pay ranges from $24.10 to $36.15 per hour based on full-time employment. Benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (subject to eligibility). Additional benefits and incentives are offered.
