Nurse Practitioner - Hospice Clinical Director
We are seeking a compassionate Nurse Practitioner to serve as the Hospice Clinical Director while providing high-quality, patient-centered care in home health and hospice settings. This role involves clinical leadership, patient assessments, treatment management, and collaboration with interdisciplinary teams to ensure optimal patient outcomes.
Responsibilities:
- Conduct patient assessments and evaluations
- Prescribe medications, manage treatment plans and develop individualized care plans
- Monitor patient progress and adjust care as needed
- Educate patients and families regarding care and treatment options
- Ensure compliance with hospice and home health regulations
- Advocate for quality patient care and positive outcomes
Qualifications:
- Current and unrestricted Idaho Nurse Practitioner license
- Experience in hospice, home health, or rural healthcare settings preferred
- APRN Certification and BLS Certification
- Ability to prescribe medications and manage treatment plans
- Knowledge of home health and hospice care practices
