As an In-Home Health Care RN, you'll have the unique privilege of building meaningful relationships with your patients, providing holistic care that extends beyond medical needs to truly touch their lives.

• Conduct thorough assessments, ensuring each patient receives personalized, comprehensive care.
• Guide and educate patients and families on health maintenance and disease prevention.
• Provide gentle assistance with Activities of Daily Living (ADLs) as needed, with dignity and respect.
• Coordinate schedule directly with patients, aligning with plan of care, approved hours, and personal preferences.
• Monitor and promptly report any changes in patient symptoms or behavior.
• Administer medications and treatments with precision and care.
• Communicate with Case Managers and collaborating physicians or specialists for unified approach to patient care.
• Maintain accurate patient logging in EMR system.
• Offer valuable advice and emotional support to patients and families.

• Prepare and provide clear, compassionate clinical notes and charting that reflect the patient's story.

• A valid and active RN license in the state of practice.
• Willingness to travel to patients' homes.
• Graduation from an accredited nursing school.
• Current BLS certification.
• Ability to pass a state and federal background check.
• Malpractice insurance is required.
• Strong problem‑solving and critical thinking skills, coupled with a warm, empathetic approach.
