Job Type: Full Time, Regular;
Non‑Exempt

Salary Range: $27.48 – $30.22 per hour (approximately $50,000 – $55,000 annually)

Shift: Monday – Friday, 9am to 5pm (with occasional evening hours as needed)

Position Summary

The Bronx County Reentry Taskforce (BCRTF), funded by the NYS Division of Criminal Justice Services, provides re‑entry services to individuals released from prison and supervised by the NYS Department of Corrections and Community Supervision. The Case Manager will conduct screenings, assessments, and counseling; develop treatment plans; facilitate Cognitive Behavioral Intervention (CBI) groups; and coordinate services with internal and external partners. The role supports clients’ stabilization and reintegration into the community.

  • Case Management – Provides individualized case management, needs assessment, service planning, and resource linkage for diverse populations.
Essential Duties and Responsibilities
  • Conduct intake interviews and needs assessments for clients.
  • Perform general counseling and case‑management duties, including weekly individual sessions, referrals, and treatment planning.
  • Assist clients in identifying and prioritizing needs; provide referrals to meet concrete needs.
  • Provide crisis intervention in collaboration with supervisor.
  • Facilitate CBI groups.
  • Ensure accurate and timely data entry of all client information into the agency’s database.
  • Provide data and information for BCRTF reports.
  • Monitor participant attendance and compliance with mandate requirements.
  • Conduct toxicology testing for clients as needed.
  • Advocate for individual client services.
  • Lead case conferences for collaboration among internal programs and external agencies.
  • Develop and maintain relationships with community‑based organizations and BCRTF stakeholders, including parole and the District Attorney’s Office.
  • Attend and participate in BCRTF meetings, conferences, and trainings.
  • Conduct weekly chart audits.
  • Engage in ongoing professional development.
  • Perform other duties as assigned.
Qualifications
  • High school diploma or GED/HSE required;
    Bachelor’s degree or higher desired.
  • Experience and commitment to working with criminal‑justice‑involved participants.
  • Strong Microsoft Office and database entry skills.
  • Strong assessment and engagement skills.
  • Strong written and verbal communication skills.
  • Bilingual English/Spanish a plus.
  • Strong organizational and leadership skills.
