Journey Care Hospice in Barrington, IL is seeking an Intake Coordinator to manage referrals, coordinate admissions, and support families with clarity and compassion.
This role provides a Monday–Friday schedule with one weekend per month. You will help schedule initial nursing assessments, maintain admission documentation, and assist outreach efforts to strengthen referral relationships within our growing care team.
